Once you have satisfied your plan specific eligibility requirements, Inland Empire Teamsters Trust will mail you a packet containing enrollment forms. Complete and return the forms to the Trust office. You will not be allowed to use the benefits until you have returned the enrollment forms. Social Security numbers are required for all individuals enrolled in the plan, including your spouse and dependent(s).
You are able to add your spouse or dependent(s) at any time. You must complete the proper enrollment forms and submit the proper documentation for either your spouse or dependent(s) or both and send to the Trust office.
Contact the Inland Empire Teamsters Trust at 509.534.0600 or 800.872.8979 and the Trust will verify your address and send them to you. You may also email your request with your current address to teamsters@rehnonline.com.
Medical claims are submitted to the local Blue Cross and/or Blue Shield Plan.
Understanding how your medical benefits work will help you make the most of your Plan. The Plan is a PPO Plan through the Premera Network. You always have the freedom to choose non-network providers but you will generally pay more when you do. That is because the PPO credentials and contracts with preferred providers to offer a network of physicians and clinics. Premera negotiates fees and passes the savings on to you. When you choose from the PPO’s network of providers, they will handle your claim paperwork. In addition, your out-of-pocket expenses will be less than if you were treated by a non-network provider since the benefits are based on discounted rates.
To find a PPO network provider, please go to www.premera.com/wa/visitor/find-a-doctor/ and click on Find a Doctor/Specialist, you will be able to enter in your doctors name to see if they are a preferred provider. You will also be able to search for a preferred provider in your area by searching by specialty or health need as well. The Teamster Trust is utilizing the "Heritage and Foundation" networks through Premera.
